The first technique of taking down a nasal hump involves cutting both the upper lateral cartilage and the dorsal septum. What can happen is the upper lateral cartilage and the dorsal septum where they meet this angle can change. It can cause some constriction of the nasal valve. Now, in some patients with a wide nasal Dorsum, this may not have any role in their breathing at all, and actually may improve their aesthetics. For other patients, this can cause over narrowing of the middle valve of the nose and can cause something called an inverted v deformity.
